Change of Major
This form must be submitted to the college to which you wish to change.
Application for Independent Study
This form must be signed by your advisor and submitted to room 336 in the Graduate Education building before you attempt to enroll.
Financial Aid Appeals
Take the completed form and all supporting documentation to your faculty advisor or advisor in the Boyer Center for Student Services. It is strongly suggested that the completed form with the advisor's recommendation be in the associate dean's office three days prior to the Financial Aid Committee meeting.
